(Due to possibility of CMS losing form please complete several forms for your signature to resubmit if necessary)

2).        CMS Test-MUST have an “A” on the test

3).        CDL-Class A with X or N endorsement

4).        Veterans-MUST supply copy of DD-214 at the time of testing

5).        Please send copy of the form the applicant receives after having tested verifying having received an “A” test to:
